What should be included:

1. Reference to MSA.
2. Scope Section.
3. Requirements Section.
4. Deliverables Section.
5. Acceptance Criteria (overall project & individual milestones/deliverables).
6. High Level Project Plan.
7. Resources On Project (by name, title, rate, and estimated hours).
8. Payment Terms - if not in MSA or if different than MSA.
9. Travel Terms.
10. Holidays.
11. Signature Block.
